Since Forever Rescue (FR)will accept animals from many
sources, including strays, we do not make any
guarantees
(including the health, temperament or mental
disposition) of the adopted animal. FR will not be
liable for any medical expenses of the adopted animal
and for any medical expenses, injury or damages to any
person or property caused in the future by adopted
animal(s), or for any causes of actions, claims,
suits, or demands whatsoever that may arise as a
result of such injury or damages pertaining to the
animal after adoption. The animals have been dewormed
and had rabies vaccination, depending on age, as well
microchip (unless other wise stated). This animal is
to be a companion animal, and treated humanely.

___ I agree to provide the animal with proper
veterinary care and adequate shelter, food and water;
and to treat the animal humanely.

report at all area animal shelter facilities.

___ I agree to give yearly reports (mail, email or
phone, and will send a photograph of this animal) of
the animal’s progress in the home and how the animal
is doing (health, behavior, ext.).

neutered at the given date Forever Rescue gives me (if
the animal is not already spayed or neutered)
___ I agree to give the the animal more than adequate nutrient ( Give the animal meat and dairies on a regular basis to keep up the protein levels,especial the taurine. This promotes the health skeletal and heart muscles.I also agree to give a vitamin D supplement to the animal to promote a strong immune function and bone health) Give nutrious humane food.
___ I agree to have ForeverRescue come to view the adjustment of the animal in 30 days with possible follow up visits.
